West Publishing has a title "Ekstrom's Licensing in Foreign and Domestic Operations" which is actually written by Robert Goldscheider -- must be a colorful history to this one. Anyway, it has lots of forms (5-volume set).

You can also try the Matthew Bender "Entertainment Industry Contracts" (10-volume set) which has a volume on books. Also Lindey on Entertainment Publishing and the Arts (5-volume set). Both contain various and sundry forms and model clauses.

For particular questions on UK law, there is a Matthew Bender title on International Copyright Law and Practice (2 vols).
