I suspect (and hope) that many of the experts on this list will suggest
better language, but I'm trying to come out of my lurker's shell and will
offer mine. For those countries, such as the U.S., where statutory moral
rights do not generally exist, I would use the proper copyright notice
followed by: "The copyright owner grants a license to reproduce this work
conditioned upon inclusion of the statement 'Produced by Andrew Sly.'"
Thus, for example, "Copyright 2001 Andrew Sly. The copyright owner grants a license to reproduce this work so long as all reproductions include the statement 'Produced by Andrew Sly.'"
And at the risk of being cynical, I might also want to specify that the attribution statement be visible, prominent, at the top or bottom of the first page, not hidden, or words of similar effect.
And hey, why not follow the shareware example and request that $10 be sent to your kid's college fund?

Subject: Wording of copyright notice.
I have a question about how to word a copyright notice.
(Or perhaps something more like an anti-copyright notice.)
I'm planning to make a series of music scores. Specifically, scores containing chorus excerpts from well-known operas. I will be using as sources, editions which are proveably public domain.
I would like to retain the moral right to be recognized as editor of the scores but explicitly give anyone freedom to make and distribute copies, as well as use for performance.
How would the following two lines be for fulfilling that purpose? Any suggestions for improvement?
Produced by Andrew Sly.
This score may be freely copied and distributed, and used for performance[,
as long as the following credit line is included on all copies: Produced by
Andrew Sly.]
