For us (university in Canada) it very much depends on for what purpose the copyright was acquired. Our campus bookstore retains the copyright permissions (paper and digital) in their offices for coursepacks sold through the bookstore. Our Distance Education Program (that's me) keeps permissions (paper and digital) for course manuals (paper and digital) in our offices. I believe the libraries keep their own permissions but I am unsure as to the details.
We are currently starting a new project which will involve digital clearance for course information mounted within password protected sites. The permissions acquired for this pilot project will be cleared and stored in paper and digital format in my office.
We regrettably do not have one office and there is a push on campus to have an office in each department and faculty...
