If you write a book and sell it and I buy a copy of it legally from a bookstore, I can create a better index than what is in your book and I can sell it to the public without the need to obtain permission from you. If it happens that I make profit on my index, there is nothing that you can do to get a share in my profit. Creating index is a separate work and it is not necessary to introduce fair use into the picture.
The issue seems to be whether it is okay for Google to make digital copy of book solely for the purpose of creating index. I think that it is okay but it is not surprised that some authors don't like that.
