I have negotiated copyright. payments and credits quite often on photo assignments and it has been mostly resolved in the form of a letter to the client (Band) or their chosen representation.
A couple of times I have had the client come back with a different type of use to the ones specified in the letter.(for example, new geographical territory) and this tends to get resolved OK like "I'll bill you and you send me a cheque within specified time frame".
I prefer the letter approach - it's fast. Legal documents that think up every possibility in advance are useful for large organisations. This is often better if the band are represented by a big company.
Most of the photographers I know like to make it as easy as possible to have their photos used and credited - without masses of frightening red tape.
