Copyright release form

If anyone has worked up a copyright transfer form, used to transfer copyright at the time of the receipt of a gift of slides or photographs, either from the actual copyright holder or the photographer's estate or heirs, I would be very interested to hear from you. I am trying to put together such a statement for our operation and would like to see what others have already done.
