Iris, book publishers generally avoid the phrase for the reasons--and others--you cite; magazine and journal publishers used it frequently in the past (given their traditional product of one edition and a single printing), however, that is changing as some periodical publishers are now publishing anthologies, on-line editions, etc. Best, Jim
