As I understand it, one time use, is literal. If for a journal one is granted rights for a specific issue. If you wish to repeat the copyrighted information then a second permission grant is required. If for a book one is granted for one edition only, regardless of the number of printings. Fee schedule is sometimes pegged to the number of projected sales. Therefore if you went into multiple printings that placed you far over the limit additional permission might be required.
