I realize this list is for copyright, but in case some readers also follow trademark issues, I thought I might mention the new TEAS
(Trademark Electronic Application System) which has just been announced
by the USPTO. See it at http://www.uspto.gov/teas/ .

For now what you can do with it is type in various information via your web browser, and it will generate a complete ready-to-file application
(including drawing) which you can print from your browser.

As USPTO describes it:

A preliminary version of TEAS which
allows you to print out an application
which has been completed and validated
over the internet, for submission to
     the USPTO via conventional (i.e.,
     non-electronic) means.
