The recently published Copyright Permission and Libel Handbook: A Step-By-Step Guide for Writers, Editors and Publishers, by Jassin & Schechter (John Wiley & Sons) has a chapter on media perils insurance. AGI and Media Professional are two insurers that write policies for self-publishers.
