sample termination policy on the web?

I am looking for sample "termination of repeat infringers" policies on the web. Does anyone on this list have urls of service provider policies posted on the web?

The Online Copyright Infringement Liability Limitation of the Digital Millennium Copyright Act requires service providers to adopt and implement "a policy that provides for the termination in appropriate circumstances of subscribers and account holders of the service provider's system or network who are repeat infringers" {section 512(i)(1)(A)}.
