Regarding your posted question:

If the copyright owner is paid the requested fee and the printer is paid for its services, it seems to me that the packager of the project can charge any fee it desires. This might not be true if, as a condition of the copyright permission, the copyright owner requested that there be some price limitation. That limitation would be in the permission agreement and may raise separate legal issues.

I just finished a book on copyright permissions and would be curious as to any other details regarding your question. Please feel free to write or call.
